Endocrinology is the study of hormones and the endocrine glands and organs that secrete them. Hormones are produced and released by endocrine glands or organs, secreted into the bloodstream and affect vital functions of our body. 


Dr. Jennifer Braimon is a board certified endocrinologist who treats all types of endocrine disorders including diabetes and lifestyle management.

Conditions treated include: 

  • Diabetes - Type 1, Type 2, Lada, Gestational Diabetes

  • Thyroid Disorders: Ultrasound Imaging available at MFM

  • Osteoporosis And Bone Health; Bone Densitometry available at MFM

Diabetes Management

Middleton Family Medicine is an American Diabetes Association Certified Program. 

The Diabetes team includes Erica Ouellette, M.Ed., RD, CDCES, Angela Fortenbacher, FNP, CDCES  and Rachel Riley, NP, CDCES & Jennifer Braimon, MD.


Individual and group counseling appointments are offered via telehealth and in the office.


Our team is up to date on the latest technology in terms of Continuous Glucose Monitors and Insulin Pumps as well as download options and ongoing support.


Our 8 week Diabetes Group Education Series is offered via Zoom and is a rolling admission.

Nutrition Counseling

Our Registered Dietitian, Erica Ouellette, M.Ed., RD, CDCES, is available to consult with you on your personalized nutrition needs including:

  • Weight management diet and education

  • Personalized meal planning including:

    • gluten free

    • food allergies

    • heart healthy

    • carbohydrate counting/carbohydrate controlled to assist with glucose control

    • gastric reflux meal planning

    • and more


Appointments are available via telehealth or in the office.
