Patient and Middleton Family Medicine Mutual Code of Conduct and Respect

At Middleton Family Medicine (MFM), we strive to provide the best care and service to our patients. We believe in fostering a respectful and positive environment for both our patients and staff. To ensure mutual respect so we may give you the best care we can, our staff pledge to adhere to the following code of conduct when interacting with our patients and we ask that our patients do the same with our staff:


  1. Respect and Courtesy:

    1. Treat each other with respect, courtesy, and kindness. Patients, please recognize that staff are here to help and support you in your healthcare journey.

    2. Use appropriate language and avoid any form of abusive, offensive, or disrespectful behavior, including derogatory comments, profanity, or threats.Patient

  2. Communication and Listening:

    1. Engage in effective and clear communication when discussing your medical concerns or needs with our staff. Provide them with accurate and complete information to assist in your care.

    2. Practice active listening. Be attentive and listen to instructions, advice, and information provided. If you have questions or concerns, express them in a polite and considerate manner.

    3. Written communication should be clear and thoughtful.

  3. Patience and Understanding:

    1. Recognize that healthcare facilities can be busy and sometimes experience unforeseen delays. Show patience and understanding if there is a wait, as MFM staff members strive to provide quality care to each patient.

    2. Understand that emergencies and urgent cases may arise, requiring immediate attention from our MFM staff members. Be patient if your appointment or procedure needs to be rescheduled or delayed in such circumstances and we will do our very best to accommodate you as soon as possible.

  4. Privacy and Confidentiality:

    1. Respect the privacy and confidentiality of other patients and MFM staff members. Do not engage in discussions or attempts to access personal information or medical records of others.

    2. Avoid sharing sensitive or personal information in public areas.

  5. Compliance with Policies:

    1. Adhere to the policies and guidelines of the MFM medical practice, including appointment scheduling, cancellations, no shows, late arrivals, and payment procedures.

    2. Follow any instructions provided by the MFM staff regarding hygiene, safety, or compliance with medical protocols and our controlled substance agreement, as they are designed to protect your well-being and that of others.

    3. For more detailed information about our policies and procedures, please ask one of our staff members.

  6. Feedback and Complaints:

    1. We welcome your feedback so that we may continue to be relentless about improving our care for you.

    2. f you have concerns or complaints about the care or service received, express them in a constructive and respectful manner. A form is available at the front desk for you to share feedback. For any urgent matters, you may request to speak with a department lead.

  7. Collaboration and Cooperation:

    1. Recognize that healthcare is a collaborative effort between patients and medical professionals. Work in partnership with our MFM staff to achieve the best possible outcomes for your health as this is our mission.

    2. Engage in open and honest communication, providing feedback, asking questions, and actively participating in your healthcare decisions.
