Remote Patient Monitoring (RPM) is a service we provide to our patients with a diagnosis of obesity, diabetes, hypertension, heart failure, chronic kidney disease and COPD.

 

Each patient is set up with a device appropriate to their diagnosis to monitor their vital signs. This service has been shown to reduce hospitalizations and provide the patient with follow up care in a more timely manner.
