At Middleton Family Medicine we believe that mental well-being is just as important as physical health, which is why we offer our patients a practice environment where they can conveniently receive all of the care under one roof.
The Behavioral Health & Addiction Services Department can provide treatment through traditional office visits which provide one to one in person support or through telemedicine visits in which you can access your provider from the comfort of your own home or office.
Individual Therapy & Medication Management
Weekly or bi-weekly therapy with Licensed Mental Health Counselor or Licensed Independent Clinical Social Worker
Medication management visits with a Board Certified Psychiatric Mental Health Nurse Practitioner
Group Therapy including:
Addiction & Recovery Support Groups
Urgent Care for patients in the midst of a Behavioral Health crisis can typically be seen within 72 hours of contact with either a Licensed Mental Health Counselor or Board Certified Psychiatric Nurse Practitioner
Individual appointments with a Board Certified Addiction Medicine Physician
Office Based Medication
Assisted Treatment including Suboxone & Vivitrol injections
