Department of Transportation Exams

Middleton Family Medicine provides DOT exams for your Commercial Motor Vehicle driver's license.

 

 When your current certificate expires, you must be examined by a medical professional listed on the National Registry of Certified Medical Examiners.

When you request your appointment, please let us know if you need a regular physical or other office visit. As this will require additional time.

 

Please note: Your DOT exam must be paid for at the time of your appointment.​​

Please bring the following documentation to your appointment:

  • When you request your appointment, please let us know if you need a regular physical or other office visit as the DOT exam will require additional time. 

  • Your DOT exam is not covered by your insurance.

    • A $150 payment is due at the time of your appointment. 

  • Please bring the following documentation to your appointment:

    • A valid Massachusetts Commercial Motor Vehicle (CMV) driver’s license.

    • A copy of your last physical exam results and/or office visit notes containing medical history (if you are not an existing MFM patient).

    • Your current Medical Examination Certificate.

Your DOT Medical Examiner will provide you with a copy of the paperwork at your visit.
